New Applicant
The Birth Care Staffing candidate needs the following Application Requirements prior to interview:
- Registered Nurse Professional License
- NRP current
- CPR by the American Heart Association - current
- Passport OR Drivers License/State ID AND Social Security Card/Birth Certificate
- ACLS, PALS and RNC (if completed)
- Health Requirements:
- Negative TB results (2-step preferred) OR Chest X-ray
- Physical Health Statement (annual exam) indicating you are in good health, free of all communicable diseases, and have no work restrictions
- Proof of Immunity
- Hepatitis B series, titer or declination
- Rubella Titer showing immunity values Or proof of 2 MMR’s
- Rubeola Titer showing immunity values Or proof of 2 MMR’s
- Mump Titer showing immunity values Or proof of 2 MMR’s
- Varicella Titer
- APPLICATION
- EEOC
- Employment Verification – Must be 3 years minimum recent experience
- Applicant Reference Release – Must be 2 separate Supervisors/Managers
- L&D Skills Checklist or
- SCN Skills Checklist
