Welcome to Birth Care Staffing's Patient Care Reporting Form. We at Birth Care Staffing operate our business in accordance with all applicable laws, rules and regulations and the highest ethical standards of fairness and honesty. All employees are encouraged to report any perceived breech of ethics or conflict of interest.
You may do so anonymously by using the form below. When submitting a complaint about an accredited organization, you may either provide your name and contact information or submit your complaint anonymously. Providing your name and contact information enables Birth Care Staffing to inform you about the actions taken in response to your complaint, and also to contact you should additional information be needed.
All completed forms are sent via email to Patty Wirth, President. Your email is confidential and will not result in any retaliation against you.
It is our policy to treat your name as confidential information and not to disclose it to any other party. However, it may be necessary to share the complaint with the subject organization in the course of a complaint investigation.
